MEDIA RELEASE AND CONSENT
Covenant Life Fellowship
I, the undersigned, hereby grant permission to Covenant Life Fellowship (the “Church”), its representatives, employees, volunteers, and authorized media partners, to photograph, video record, and/or otherwise capture my signed-up child or self image, likeness, voice, and appearance during VBS services, events, and activities.
I further grant Covenant Life Fellowship the irrevocable right and permission to use, reproduce, edit, publish, distribute, and display such photographs, video recordings, or other media in any format or medium now known or later developed. This includes, but is not limited to, use on social media platforms, websites, livestreams, promotional materials, advertisements, publications, and broadcasts, for purposes including ministry, promotion, education, and outreach.
I understand that:
My child's and my participation is voluntary, and I will receive no financial compensation for the use of such media.
Covenant Life Fellowship owns all rights to the media captured and may use it without further notification or approval.
The Church is not responsible for unauthorized use of images or recordings by third parties once such materials are published publicly.
I waive any right to inspect or approve the finished product in which my likeness appears and release Covenant Life Fellowship from any liability arising from the use of such media, including any claims for libel, invasion of privacy, or infringement of publicity rights.
If the undersigned is a parent or legal guardian of a minor, I hereby grant permission for the minor named below and agree to all terms stated in this release on their behalf.
